Description

The DRG Validation Auditor extracts clinical information from a variety of medical records and assigns appropriate procedural terminology and medical codes (e.g., ICD-10-CM and PCS) to patient records. The Medical Coding Auditor work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.

Responsibilities

The DRG Validation Auditor confirms appropriate diagnosis related group (DRG) assignments. Analyzes, enters and manipulates database. Responds to or clarifies internal requests for medical information. Understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas. Makes decisions regarding own work methods, occasionally in ambiguous situations, and requires minimal direction and receives guidance where needed. Follows established guidelines/procedures.

ABOUT THE ROLE

A modern approach to weight requires modern practices, and that means radically remodeling how intensive lifestyle intervention programs operate and scale. As part of our clinical program team, you’ll deliver several critical components of the Calibrate program: outreach and support for members to enhance their program progress, clinical program coordination, and confirmation of clinical appropriateness for clinical care pathways, introduction to our method, and setting program plans with members.

This position will assist the manager and supervisor in training new team members, coordinate inquiries from ancillary departments regarding DNFB and edit tasks. The Sr. Edit Coder will investigate and solve edit issues and communicate root cause data to management in order mitigate potential upstream and downstream impacts.

Responsible for modifying and completing moderate to high complexity reviewing and resolving coding and charge edits using ICD-10-CM, CPT and HCPCS or any other designated coding classification system in accordance with coding rules and regulations. ESSENTIAL DUTIES AND RESPONSIBILITIES

• Performs claim edit reviews on outpatient encounters to validate appropriateness of the CPT codes, HCPCS Level II codes, and modifier assignments, APC group appropriateness, review for missed secondary diagnoses and/or procedures, and ensure compliance with all APC mandates and outpatient reporting requirements. Monitors medical visit code selection by departments against facility specific criteria for appropriateness. Assists in the development of such criteria as needed. Addresses CCI and LCD edits within the various billing editors while abiding by the Standards of Ethical Coding as set forth by the American Health Information Management Association. Meets and/or exceeds Conifer’s Edit Coder productivity standards.
• Runs and submits coding operational reports to leadership as requested, reviews data and identifies opportunities or trends. Demonstrates working knowledge of DNFB and uses data to drive performance excellence. Ability to analyze, display, and communicate data in meaningful manner. Ability to maneuver thru various electronic systems effectively.
• Ability to deal with customer/partner issues and resolve conflict. Ability to multi-task and meet deadlines. Will act as a resource for Edit Coders.
• Reviews claim denials and utilizes the medical record in determining accurate code assignment of all documented diagnoses and procedures adhering to the standards of ethical coding.
• Monitors DNFB report for outstanding and/or uncoded encounters to ensure timeliness of coding completion. Brings identified issues to department managers for resolution.

Summary:

The RN Specialist reports to the Senior Manager of Surgical Quality & Clinical Operations and is responsible for guiding members through their Transcarent experience in partnership with Transcarent’s Live Services Department. Responsibilities:

Demonstrates the ability to perform quality coding on ancillary charts, clinic charts, and emergency room records.

Maintains a working knowledge of ICD-10 and CPT coding principles, governmental regulations, official coding guidelines, and third-party requirements regarding documentation and billing. Provides accurate answers to physician’s/hospitals coding and/or billing questions within eight hours of request. Responsible for coding or pending every chart placed in their queue within 24 hours. It is the responsibility of the coder to notify administrative staff in the event they cannot meet the twenty-four hour turn around standard. Coders are responsible for checking the Guidehouse email system at least every two hours during coding session. Coders must maintain their current professional credentials while working for Guidehouse. Coders are responsible for becoming familiar with the Guidehouse coding website and using the information contained in the website as a daily tool to correctly code and abstract for each facility. As an integral member of the VillageMD Team, the Clinical Documentation and Coding Accuracy Educator is accountable for ensuring providers are documenting and coding conditions in accordance to the VMD standard. The goal of the role will be to oversee provider education and subsequently improve documentation accuracy across HCC coding.

The Clinical Documentation and Coding Accuracy Educator will review performance metrics and reports, as well as patient charts, to identify areas of opportunity to support coding accuracy and effective documentation practices. He/she will educate all primary care providers, physicians and advanced practice practitioners, and other clinical staff on a process for improving coding accuracy performance, proper documentation and general coding practices.

Education

• Conduct inidual training and group education sessions on proper coding and documentation practices for physicians and staff consistent with industry standards and in compliance with coding guidelines

About the Billing Support Specialist Role

Upperline Health is seeking a Billing Support Specialist to support a team of clinicians in delivering complex health services by providing telephone billing support and assistance to patients and team members across all our podiatric clinics. Ideal candidates will have prior experience working with medical billing, familiarity with medical terminology and insurance as well as customer service experience. This person must be an outstanding communicator, and a team player who demonstrates strong attention to detail and thrives in a collaborative environment. What You’ll Do

• Effectively triage inbound medical billing related calls by asking appropriate fact-finding questions
• Assist in resolving billing related questions and issues with patients in a timely and appropriate manner over the phone
• Adhere to all company policies and procedures regarding payment arrangements, account documentation, proper disclosures, and update of patient information
• Answer internal medical billing related questions from team members working in the clinics in addition to the questions from patients directly
• May be requested to support and perform other duties based on business demand

Qualifications of the Billing Support Specialist

• Exceptional customer service orientation featuring an empathetic, compassionate and professional demeanor with each interaction
• Significant familiarity and experience with medical billing (HCPCS, ICD-10), medical insurance, and medical terminology
• Tenacious problem solver, with demonstrated capacity to embrace complex problems and arrive at effective solutions in a timely manner
• Experience with Athenahealth EMR is ideal but not required
• Enjoys working in a team-based environment with active collaboration
• Must be an effective communicator, able to explain billing resolutions in an informational, influential, concise, and personable manner with outstanding etiquette
• Strong written communication skills with success in providing notes, updates, and written communications via computer systems
• Thrives in a fast-paced environment that relies on the ability to multi-task and balance multiple, competing priorities, yet is still able to balance that energy and drive with sensitivity and compassion

Qualifications

• Advanced degree in a life science (PhD or PharmD preferred);

At this time, we are looking for a PRS (Patient Record Specialist) who will be primarily responsible for supporting the documentation of provider activity for billing and clinical quality Successful candidates will possess a sense of urgency, accuracy, and a commitment to daily tracking and reporting. The PRS will report to one of Forefront’s Director of Care Delivery for inpatient services or emergency services.

Duties and Responsibilities include:

1. Confirm that daily encounters are properly input into Sales Force tracker platform
2. Collect face sheets for all patients seen and store in FFT EMR
3. Complete Patient Registration in FFT EMR capturing demographic data efficiently, and accurately from Hospital EMR for all patients seen
4. Insurance verification and documentation.
5. Retrieve Discharge Summaries from hospital EMR and Upload into FFT EMR
6. Responsible for all Facility Requested Providers signatures through Docu-Sign
7. Support Data Entry for Monthly Facility Schedules
8. Support Unique workflows for facilities as needed
9. Communicates verbally and electronically with providers, facilities, and others
10. Maintain confidentiality of patient information
11. Other duties as business needs dictate

Requirements

1. Excellent prioritization, decision-making and multi-tasking skills are essential
2. Resourceful to secure necessary documentation
3. Superior attention to detail and accuracy
4. Experience with EMR systems; ability to quickly learn new software systems
